I am not sure whether it helps, but I haven’t noticed it again. So all 3 times I noticed it happened while I was TL4 and category moderator. This could of course be because I have spent more time here, but my feeling is that it is more likely related to the extended permissions (review queue and unlisted topics come to my mind first)
Unfortunately, I only have a screenshot in one case that actually shows more than what I have posted here. There I can see that the Contribute > Feature topic where the duplicated related tab showed was a spam topic, which I had probably just flagged. Additionally I can see one pending post to review in the sidebar, which has to be a different topic because I wasn’t a category moderator for Contribute > Feature.
