If this is on another forum, could you send a link to that forum? It might be easier to see it in action rather than a screenshot.
I cannot recall any built-in feature that does this. It is likely a plugin/Theme Component.

Not really – that’s a policy widget that displays at the end of a post that you want people to acknowledge. Is there a reason the form templates solution won’t work for you?

You can use Custom Wizard for this or even User Fields.
Both of those solutions integrate with Discourse.
