I cannot see where /var/discourse/config/multisite.yml on the host would automatically be mounted or copied to /var/www/discourse/config/multisite.yml inside the app container.

You’d need to add something like this to your app.yml

  - volume:
      host: /var/discourse/shared/multi/multisite.yml
      guest: /var/www/discourse/config/multisite.yml
