We believe we have seen this before - it sounds like [this problem](https://meta.discourse.org/t/discourse-bug/384684/23).

Do you have any logs (see the `/logs` path as admin) indicating what the error it received was? We’ve recently improved SMTP error handling and hopefully it will give us enough information to solve the problem.
