Users who are 'activated' but not 'approved'

Our site requires staff approval for joining the community. But on several occasions, we have come across users whose profiles showed that they were Activated - Yes but Approved - No. These users would also show up in blue, not grey, on our admin-side Users list.

Activated means they confirmed their email address, and approved means a staff member confirmed the account in the review queue.
