This appears to be happening lately. There is no exact way to produce this bug, it just happens lately:
I am watching the ban appeals category in https://forum.dark-gaming.com/ and this bug seems to be happening sometimes. (This category usually requires approval)
I’ve slipped this over to support as there aren’t very clear repro steps, and it may be site-specific. If you have any more details we can always move it back again.
