Boxes are display incorrectly without the icon without the description and without and posts as per below:
I fixed the issue by deleting and recreating.
Possible causes:

I created the boxes before a migration into another hosting environment
-
They were initially hidden immediately upon creation
No matter what settings I changed I could not get them to work. Deleted boxes and recreated and did not have any issues since then.
