There was an issue recently where having the old discourse-feature-voting git along with post-voting in your app.yml would cause error 500 for the whole site. This is slightly different though, but may still be worth a check?
If so, could you try updating your app.yml with the discourse-topic-voting and discourse-post-voting links and see if that helps?
