I see Discourse Akismet (spam filter) costs about $500 a year. Apart from this, do they charge anything more? Also, does the product catch spam before it is visible on the community? How does it work?

I don’t think so. AFAIK the API key comes with the plan, and the plugin doesn’t cost $.
