Get 2 emails for the same reply - user_posted & user_mentioned

What version are you running?

I “remember” this being mentioned here on meta and fixed recently.

This was only partially fixed by @zogstrip’s fix, I added a more atomic fix here: https://github.com/discourse/discourse/commit/0119a2f9800de5a17d4141a029a1bac21c491fd0 In particular “ninja” editing was able to trigger dupes
