How handle a long topic create form

Please see the screenshot. Functionally it is working if I drag the create window up to the top. However from UX perspective, I need to resolve this. Any suggestions on how to handle a really long create form?

Are you using your own plugin or a existing one? Or is this the form-templates feature?
