Has anyone put a Hotjar script on their Discourse forum before? I’ve someone from my company asking me if we can run it to run a poll and gather some feedback, I’d also love to know (if it’s possible) where to place the script?

I do not use it but if it is a tracking script like google analitycs you can put it in /admin/customize/themes inside the /head tab. Then remember to add this new theme as a component of your main theme.
