Did something go awry and they never received it? Does the PM show up in their inbox? (And can you see the notification for it on one of the user’s notifications page?)

Good idea, yes on their notification page the PM is there. I am thinking of a setting par user (by default) which would prevent them from being notified ?

How about the emails? Anything in your email logs to confirm a notification was sent out or skipped? Was it to 562 individuals, or a message to a group? When you spot check a few, have they definitely visited the site recently enough to have been able to see the PM?
