Yes, this is possible — we have this currently setup in our Contribute > Feature category here on Meta, using the Discourse Topic Voting plugin If you want to structure the topics like a form, you might also want to try out Introducing Form Templates

In addition to the plugin, you can also use Alternative Voting Category Style
