The topics in my example was initially «on hold» and approved by a moderator:

I’m confused…that setting simply allows topics to have an identical title to another topic, it has nothing to do with whether a topic is held for review. I fail to see how topics held for approval is connected to duplicate titles.
I think his report is a bit different, he’s saying even if dupe topic titles are disallowed, they can inappropriately come through on approval.

We had a similar question from a customer, and I believe the moderation queue bypass most validations, since you are guaranteed that a moderator said that the topic was okay.
